Job Title: Nurse Practitioner

SAC Health is seeking a skilled Nurse Practitioner to join our team in Barstow, CA. As a Nurse Practitioner, you will play a vital role in delivering high-quality healthcare services to our patients.

Job Summary:

The Nurse Practitioner will work collaboratively with our interdisciplinary team to provide comprehensive patient care, including advanced treatment plans, medication management, and specialist referrals. You will be responsible for coordinating outpatient care, performing triage, and providing patient education to patients and their families.

Key Responsibilities:
  • Apply advanced nursing knowledge and skills to manage call queues, perform triage, and coordinate patient referrals.
  • Develop and implement educational programs tailored to the specific needs of patients, families, and healthcare professionals.
  • Serve as a clinical resource and provide expertise to clinic staff, physicians, patients, and other departments on medical and nursing matters.
  • Conduct comprehensive nursing assessments and perform diagnostic and therapeutic procedures as required.
  • Document patient encounters accurately and efficiently in the EMR system.
  • Actively participate in quality improvement initiatives aimed at enhancing the quality of care provided by the NP.
Requirements:
  • Master's Degree in Nursing and completion of NP curriculum required.
  • Active and unrestricted California Registered Nurse (RN) license and California Nurse Practitioner Certificate required.
  • Minimum one year experience in an area of NP specialty required.
  • Proficiency in EMR systems like Epic, advanced computer skills, and effective telephone use.
  • Strong interpersonal abilities to communicate effectively with patients, families, healthcare professionals, and interdisciplinary teams.
