We are seeking a compassionate and skilled Registered Nurse to join our team as a RN Case Manager. As a RN Case Manager, you will play a vital role in supporting patients and families facing end-of-life journeys.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Coordinate with interdisciplinary teams to ensure comprehensive care
  • Provide regular visits to patients and families to assess needs and update care plans
  • Perform clinical pronouncements of death and provide support to families
  • Maintain accurate patient records and charts

Requirements:

  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Current CPR and First Aid Certification
  • Knowledge of quality assessment and assurance procedures
