Key Responsibilities:
  • Conduct psycho-social assessments to identify patients' needs and develop personalized care plans.
  • Provide counseling services, crisis intervention, and support to patients and families coping with substance use disorders.
  • Collaborate with healthcare teams to ensure comprehensive care and continuity of services.
  • Maintain a network of community resources to support patients' transition back to their communities.
Requirements:
  • Master's Degree in Social Work or Counseling.
  • Two years' experience in clinical social work.
  • State of Florida Licensed Clinical Social Worker (LCSW) certification.
Preferred Qualifications:
  • Experience in acute care hospitals.
  • Working with medically complex patients.
  • Substance use disorder experience.
