Job Summary and Qualifications
The Case Manager (CM) is responsible for ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. The CM coordinates and directs care tailored to the specific needs of each rehabilitation patient, collaborating with the Rehab Program Director and Facility Case Management Director.
We are in search of a Registered Nurse Case Manager to help us maintain our commitment to delivering exceptional care.
Key Responsibilities:
- Provide program orientation to patients, families, and caregivers, covering the role of the case manager, rehabilitation philosophy, and discharge criteria.
- Act as the primary coordinator for patient and family education, encouraging their involvement in discussions about care plans and goals.
- Ensure the implementation of individualized treatment plans that align with the patient's strengths and preferences.
- Document findings from discharge planning evaluations and psychosocial assessments, communicating any needs or preferences to the care team.
- Promote positive patient interactions that enhance the overall patient experience.
Qualifications:
- Registered Nurse with a valid nursing diploma.
