Position Overview

The Registered Nurse Care Coordinator in the Ambulatory Clinic is a licensed professional who excels in delivering ethical, evidence-based, and compassionate nursing care. This role emphasizes patient safety and the enhancement of nursing quality by assessing and addressing patient needs, executing clinic-related care, providing health education, advocating for patients, coordinating healthcare services, and evaluating patient outcomes.

Key Responsibilities

  • Oversee patient flow management, triage, and direct care.
  • Act as a communication liaison among front desk personnel, nursing staff, physicians, and clinic management.
  • Manage patient care by establishing expected outcomes, defining resource utilization, and collaborating with providers to set patient goals.
  • Deliver nursing support and coordination to optimize patient outcomes and enhance quality of life through clinical and educational initiatives.
  • Encourage self-management of health conditions among patients.
  • Ensure continuity of care for patients and families post-hospital admission, discharge, and emergency visits.
  • Handle high-risk patient care, particularly for those with multiple co-morbidities or at risk for hospital readmission.
  • Build rapport with patients and utilize assessment skills to identify their physical, psychosocial, financial, and environmental healthcare needs.
  • Provide education to patients and relevant staff regarding healthcare needs, safety concerns, and benefits of patient education.
  • Utilize electronic medical records (EMR) for efficient documentation of assessments, clinical findings, treatment plans, and progress towards goals.
  • Monitor, document, and report on patient conditions and responses to treatments and medications.
  • Maintain and review patient records, ensuring accurate documentation of tests and examination results.
  • Respond to patient inquiries related to treatment and provide guidance on medications and care instructions.
  • Uphold patient confidentiality at all times.
  • Document patient care activities using EMR in accordance with practice standards.
  • Demonstrate professional, clinical, and technical competencies relevant to the assigned unit or department.

Required Qualifications

  • High School Diploma
  • Bachelor's Degree in Nursing
  • Minimum of 2 years of RN experience

Preferred Qualifications

  • Experience in a medical office setting

Licensure and Certification Requirements

  • Basic Life Support Heartsaver (BLS HS) certification (Non-Clinical) from the American Heart Association (AHA) or obtained within 14 days of hire
  • Licensed Registered Nurse (LRN) with a multi-state license from the State Board of Nursing

Preferred Licensure and Certification

  • Active multi-state Registered Nurse (RN) license in good standing or an active single-state RN license with proof of application for a multi-state license.

Knowledge and Skills

  • Proficient in typing and word processing
  • Strong analytical, critical thinking, and problem-solving abilities
