Job Summary

We are seeking a highly skilled and experienced MDS Nurse to join our team at McCall Rehabilitation and Care Center. As a key member of our interdisciplinary team, you will play a critical role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plans.
  • Participate in facility surveys and inspections made by authorized government agencies.
  • Support Quality Assurance and Performance Improvement (QAPI) initiatives to develop and implement plans of action to correct identified deficiencies.
  • Coordinate the interdisciplinary team to ensure timely completion of assessments and care plans.
  • Utilize the MDS Scheduler to plan and set Assessment Reference Dates (ARDs) and communicate ARD selection to the IDT.
  • Accomplish MDS sections as assigned and fulfill CAAs as required.
  • Complete Care Plan items and attend Care Plan meetings as assigned.
  • Implement monthly infection tracking logs and conclude UDA's during observation periods.
  • Finalize diagnosis coding and create diagnosis sheets, confirming skilled physician's certification.
  • Monitor documentation in the medical record to validate that it supports MDS coding.
  • Participate in skilled utilization and State Case Mix management as assigned.
  • Deliver teaching and training for MDS item completion to interdisciplinary team members.
  • Provide teaching and mentoring of CNA staff on Nursing ADL documentation.
  • Sign off on Minimum Data Set (MDS) for each resident within the facility.
Requirements
  • High School Diploma or equivalent.
  • Minimum one year of experience as a nurse in a hospital, long-term care facility, or other related healthcare facility.
  • Active license to practice as a RN/LVN/LPN license valid in this state.
  • Experience with RAI process and MDS certification preferred.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions and deal tactfully with personnel, residents, family members, visitors, and government agencies.
