Job Summary

We are seeking a skilled Registered Nurse PC to join our team. As a Registered Nurse PC, you will be responsible for providing skilled nursing care to patients in their homes, including assessment, instruction, and management of care plans.

Key Responsibilities
  • Confer with physicians to develop initial plans of treatment based on patient assessments and physician orders.
  • Provide hands-on care, management, and evaluation of care plans, and teach patients in accordance with physician orders.
  • Admit patients to services, completing clinical documentation under the Clinical Manager's direction.
  • Assess individual patient status and care/instruction needs.
  • Coordinate and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers.
Requirements
  • Bachelor's or Associate's degree in Nursing or equivalent.
  • RN licensure.
  • Proof of current CPR certification.
  • At least six months of professional nursing experience as a Registered Nurse.
