About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Home Health Care Specialist. As an integral part of our patient care team, you will provide high-quality nursing care to medically fragile children and adults in their homes.

Key Responsibilities:
  • Provide skilled nursing care to patients in their homes, including assessments, treatments, and education on self-care techniques and prevention strategies.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Maintain knowledge of current medical practices and policies by participating in continuing education programs and workshops.
Requirements:
  • Graduate of an accredited school of nursing with a valid LPN/LVN license in the state of practice.
  • Current CPR certification and TB skin test (within the last 12 months).
  • Six months prior hands-on nursing experience preferred, but not required.
  • Must have reliable transportation and be able to travel to patient homes.
