Optimus Health Care is a Federally Qualified Health Center with multiple locations in Connecticut. We are the largest provider of primary healthcare services in Fairfield County, committed to delivering high-quality care to all patients, regardless of their ability to pay.
Job Summary:The Registered Nurse coordinates primary care services for patients from diverse populations in a professional manner. This role works closely with the Practice Manager, Director of Nursing, Nurse Manager, and medical providers to ensure compliance with Optimus' standards of care.
Key Responsibilities:- Promote and restore patients' health by implementing the nursing process, collaborating with physicians and multidisciplinary team members, and providing patients with physical and psychological support.
- Promote patients' independence by establishing patient care goals, teaching, and responding to patients' questions regarding their medical condition, medications, and self-care skills.
- Ensure quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and standards, and making or recommending necessary adjustments.
- Facilitate the clinical orientation for new hires and provide performance feedback as needed.
- Assist the Nurse Manager in daily oversight of the LPN and MA, report clinical and operational issues, and collaborate with the Practice Manager and Nurse Manager to keep clinical staff informed of changes.
- Work with providers to coordinate care for patients and provide clinical support, resolve patient concerns, and monitor tracking systems for patient results.
- Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
- Valid and current RN License to practice in the State of Connecticut.
- CPR certification required upon hire and recertification bi-annually.
- Ability to work with staff with different skill levels and diversity.
- Excellent spoken and written communication skills.
- Excellent leadership and motivational skills.
- Ability to build relationships with colleagues at all levels.
- Ability to plan and prioritize own work and that of others when needed.
- Bachelor's Degree in Nursing.
- Minimum of one to two years of experience as an RN or prior experience as an LPN in a community health setting.
- Familiarity with tele-medicine and prior experience a plus.
