Provide routine health screenings and counseling on lifestyle changes
Guide and coordinate all aspects of care as an engaged and active member of a multi-disciplinary treatment team
Work with the team to achieve the best possible patient outcome in the most cost-effective manner
Requirements:
BC/BE Family Medicine
Ability to hold a CT state medical license
Benefits:
