The ideal candidate will have a minimum of 5 years of work experience post-graduation of an accredited school of nursing and a current state Registered Nurse license. Grandfathered prior to April 1, 2015, or a minimum of 5 years post-graduate of an accredited school of Social Work for Licensed Clinical Social Worker. However, RN Case Manager is preferred.
The Case Manager RN will work on behalf of the advocate, promoting cost containment and demonstrating leadership to integrate healthcare providers to achieve a seamless delivery of care. The methodology is designed to facilitate and insure the achievement of quality, clinical and cost-effective outcomes and to perform a holistic and comprehensive admission and concurrent review of the medical record for the medical necessity, intensity of service and severity of illness.
Responsibilities:
- Conduct comprehensive assessments of patients to identify their healthcare needs and develop individualized care plans.
- Collaborate with healthcare providers to ensure seamless delivery of care and promote cost containment.
- Develop and implement strategies to improve patient outcomes and reduce healthcare costs.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and compliance with care plans.
Qualifications:
- Current state Registered Nurse license.
- Minimum 5 years of work experience post-graduation of an accredited school of nursing.
- Grandfathered prior to April 1, 2015, or a minimum of 5 years post-graduate of an accredited school of Social Work for Licensed Clinical Social Worker.
- Knowledge of Milliman Criteria and InterQual Criteria preferred.
- Experience and knowledge in basic to intermediate computer skills.
