STGi is currently in search of a Licensed Social Worker to deliver essential services within a Community Based Outpatient Clinic.

This clinic operates during standard business hours from Monday to Friday.

The primary role of the Medical Social Worker (MSW)/Licensed Social Worker (LSW) involves providing social work services under the guidance of the clinic's healthcare team, including physicians, psychiatrists, nurse practitioners, and management representatives.

Key Responsibilities:

  • Deliver social work services based on medical directives to address the physical, mental, and emotional needs of patients.
  • Conduct initial assessments for adult and geriatric patients in Primary Care and/or Psychiatric settings.
  • Engage in treatment planning, discharge planning, referrals, and patient placements as necessary.
  • Provide support for patients with substance use disorders when applicable.
  • Administer specialized psychotherapeutic interventions for patients and families facing complex psychosocial challenges.
  • Facilitate referrals for various therapeutic services including individual, group, marital, family, financial, or activity therapies.
  • Execute mandatory reporting duties, including Child Protective Services (CPS), Adult Protective Services (APS), and Elder Abuse.
  • Offer crisis intervention, supportive counseling, and problem-solving strategies as needed.
  • Collaborate as part of a multidisciplinary treatment team to evaluate treatment needs and develop strategies and goals.
  • Monitor treatment progress and provide constructive feedback to the treatment team.
  • Act as a liaison between patients' families and the healthcare team.
  • Serve as a community liaison with VA and mental health agencies.
  • Provide resources and information to other clinic staff regarding community services.
  • Facilitate group therapy sessions in compliance with state regulations.
  • Uphold infection control practices and maintain patient safety protocols.
  • Ensure a smooth patient flow in accordance with established access standards.
  • Perform CPR and assist in emergency medical situations as required.
  • Document all relevant patient information accurately in medical records to ensure quality care and continuity.
  • Maintain tracking systems for patient reminders and appointments.
  • Educate patients and families about their care plans and available resources.
  • Assist in ensuring accurate documentation related to patient access and wait times.
  • Participate in performance improvement initiatives with the healthcare team.
  • Complete all necessary documentation and reports in a timely manner.
  • Support compliance with all relevant regulations and standards.
  • Maintain confidentiality and uphold patient privacy rights.
  • Perform additional duties as assigned.


Required Qualifications:
  • Master's degree in Social Work from an accredited institution.
  • Certification in psychotherapy.
  • Current state licensure as an MSW/LCSW.
  • Current CPR/Basic Life Support certification.
  • Proficient knowledge of social service principles and techniques.
  • Understanding of crisis intervention and family dynamics.
  • Ability to assess patient conditions and respond effectively in emergencies.
  • Capability to set measurable educational goals for patients.
  • Experience in providing individualized and group counseling.
  • Familiarity with customer service principles for diverse populations.
  • Strong verbal and written communication skills.
  • Proficient in medical terminology and basic computer skills.


Preferred Experience:
  • At least one year of experience working with mental health patients.
  • 2-3 years of experience as an MSW/LCSW.
  • Must be a US Citizen.
