About the Role:

Amedisys, Inc. is seeking a highly skilled and compassionate Registered Nurse to join our team as a Hospice Care Specialist. As a key member of our home health care team, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Responsibilities:

  • Perform initial nursing assessments on new patients to identify their unique needs and develop personalized care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination and continuity.
  • Communicate patient progress to the clinical manager, physician, and care team to ensure informed decision-making.
  • Develop, prepare, and maintain accurate and timely patient care progress records.
  • Educate patients and their families on condition management, care needs, and available resources.
  • Make referrals to other hospice care disciplines as needed, or document rationale for not doing so.
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned.

Requirements:

  • 1+ year of nursing experience required.
  • Current RN license in PA.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
