Estimated Salary: $55,000 - $65,000 per annum, depending on experience.

Job Description:

The Senior Patient Advocate provides expert psychosocial support to residents and their families, ensuring they receive the highest level of care and attention. This involves performing resident assessments, creating and updating care plans, and coordinating resident visits with outside services.

Key Responsibilities:

  • Provide direct psychosocial intervention to residents and their families.
  • Conduct thorough resident assessments at admission and annually, identifying areas for improvement and developing targeted care plans.
  • Collaborate with healthcare professionals to ensure seamless care delivery and effective communication.
  • Supervise and guide Social Services Assistants to ensure optimal performance and outcomes.

Requirements:

To succeed in this role, you will need:

  • A Bachelor's degree in Social Work (BSSW) or Master's degree in Social Work (MSSW).
  • Licenses/Certification: Licensing as required by the state of employment.
  • At least one year of experience in a long-term care environment.
