About the Role:

Cogir Senior Living is seeking a compassionate and detail-oriented Medication Care Partner to join our team at Tribute at Melford. As a Medication Care Partner, you will play a vital role in providing direct care to our senior residents, ensuring their medication and treatment plans are followed in a unique and supportive setting.

Responsibilities:

  • Assist residents with daily medication administration, including self-administration.
  • Keep charting documentation current and report resident status changes.
  • Coordinate medication orders and deliveries with pharmacies.
  • Keep medication files and medical records current and up to date.
  • Provide compassionate care and support with activities of daily living to residents in a team setting as needed.
  • Prioritize independence and provide emotional support to residents by offering continuous engagement and companionship.
  • Encourage and assist with participation in life enrichment activity programs.
  • Promote open communication between healthcare professionals, families, residents, and staff.

Requirements:

  • Positive team player attitude and love working with people.
  • Excellent verbal and written communication skills.
  • A minimum of 1 year of caregiving experience, including giving medication in a community setting.
  • Experience with Memory Care is a plus.
  • A current state Certified Medication Tech (CMT/CMA) license.
  • A current state CNA license.
  • Current First Aid and CPR certificate or ability to obtain.
  • Previous experience working in Independent Living (IL), Assisted Living (AL), Memory Care (MC), or Senior Living experience is a plus.
  • High School Diploma or equivalent.
