Job Overview

The Advanced Practice Provider plays a crucial role in fulfilling the mission and vision of Neighborhood Healthcare by delivering exceptional patient-focused care and services within a collaborative team setting. This position is responsible for providing direct healthcare services to patients under the supervision and accountability of Physicians, focusing on three primary areas: direct patient care, team collaboration, and clinical management.

KEY RESPONSIBILITIES

DIRECT PATIENT CARE

  • Conducts thorough physical assessments on patients, including evaluation of medical histories.
  • Orders, administers, and interprets diagnostic tests, including x-rays, electrocardiograms, and laboratory analyses.
  • Executes therapeutic procedures such as injections, immunizations, suturing, and wound management.
  • Educates and advises patients on adherence to prescribed treatment plans, developmental milestones, family planning, and overall health maintenance.
  • Enhances patient experiences through courteous interactions and a commitment to precision.
  • Provides age-appropriate care tailored to individual patient needs.

TEAM COLLABORATION

  • Shares responsibility for patient health outcomes, collaborating effectively with care teams.
  • Instills confidence in our care and facilities among patients, colleagues, and stakeholders.
  • Responds promptly and compassionately to the needs of patients and staff.
  • Maintains professional relationships with all levels of personnel, clients, and the community.
  • Contributes to achieving departmental goals and objectives.

CLINICAL MANAGEMENT

  • Develops and implements patient management strategies, documents progress notes, and aids in ensuring continuity of care.
  • Operates in accordance with NHC practice protocols under the guidance of supervising MD(s), adhering to the Supervisory Agreement.
  • Stays informed about organizational initiatives and promotes the mission and objectives.
  • Functions at the highest level in accordance with credentials and competencies.

EDUCATION AND EXPERIENCE

  • Master's degree from an accredited physician assistant or nurse practitioner program is required.
  • Current CA Physician Assistant or Nurse Practitioner certification is required.
  • Valid CA DEA license is required.
  • Current BLS certification from an approved American Health Association provider is required upon hire.

ADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities)

  • Exceptional verbal and written communication skills, including strong composition, typing, and proofreading abilities.
  • Ability to interpret various instructions in written, oral, diagrammatic, or scheduled formats.
  • Knowledgeable and experienced in providing direct patient care.
  • Capable of managing multiple tasks simultaneously.
  • Excellent planning and organizational skills.
  • Able to work collaboratively within a team as well as independently.
  • Proficient in handling highly confidential information in a professional and ethical manner.

PHYSICAL REQUIREMENTS

  • Ability to lift and carry up to 20 lbs.
  • Ability to stand for extended periods.

COMPLIANCE (Safety & HIPAA)

  • Adhere to all safety protocols as outlined in Neighborhood Healthcare's Illness and Injury Prevention Plan (IIPP) and report any injuries or unsafe conditions immediately.
  • Maintain current knowledge of policies and procedures related to safe work practices.
  • Follow all safety procedures and report unsafe conditions.
  • Utilize appropriate body mechanics to ensure a safe working environment.
  • Be familiar with the location of the nearest fire extinguisher and emergency exits.
  • Comply with all infection control protocols, including blood-borne pathogen guidelines.
  • Ensure the privacy of all patient, employee, and volunteer information, accessing such information only on a need-to-know basis for business purposes.
  • Adhere to all regulations regarding corporate integrity and security obligations.
  • Report any unethical, fraudulent, or unlawful behavior or activities.

Compensation Range: $147,000 to $199,000 per year, based on experience.
