Job Summary:

As a key member of the Kaiser Permanente team, the Supervisor of Home Health Services will oversee the planning, development, implementation, and evaluation of rehabilitation therapy programs and services provided within the Care at Home, home health, palliative, and hospice service lines. This role will collaborate with the community agency division to ensure seamless delivery of rehabilitation therapy services that meet or exceed standards for cost, quality, performance, access, member satisfaction, and clinical/functional outcomes.

Key Responsibilities:
  • Program Development and Implementation: Supervise the planning, development, implementation, and delivery of integrated continuum of rehabilitation therapy services and programs for members eligible for home health, palliative care, hospice, and extended care benefits.
  • Performance Monitoring and Evaluation: Design and utilize systems, methodologies, and processes to monitor, evaluate, and continuously improve rehabilitation therapy functional outcomes, services/programs, and utilization, cost, clinical, member satisfaction, and performance measures/standards.
  • Staff Management and Development: Supervise the performance and competency evaluation process for rehabilitation therapy professional staff; identify and provide for professional development needs; hire, promote, fire, and discipline staff; evaluate clinical performance of staff and provide clinical supervision and direction.
  • Financial Management: Develop, justify, and monitor financial, capital, and FTE budgets within the care at home rehabilitation department.
  • Regulatory Compliance: Ensure compliance with health plan, Medicare, Title 22, TJC, and all other pertinent local, state, and federal regulatory agency requirements.
  • Quality Improvement: Oversee the evaluation, modification, and improvement of existing programs and outcomes; determine feasibility of new programs to meet anticipated service needs.
  • Collaboration and Communication: Collaborate with patients, physicians, other departments, organizations, and agencies to address and resolve patient care, transfer/discharge, and program service issues.
  • Expert Resource: Act as a clinical practice expert/resource to staff.
  • Quality Assurance: Ensure patients receive quality care by reviewing documentation, consulting with staff, reviewing care plans, conducting/participating in multidisciplinary team conferences, and reviewing patient satisfaction reports.
  • Field Visits: Conduct minimum quarterly field visits with Home Health staff.
Requirements:
  • Experience: Minimum five (5) years of clinical rehabilitation therapy experience in an acute, home health, outpatient, or community health setting.
  • Education: Graduation from an accredited physical therapy, occupational therapy, or speech language pathology educational program with a Bachelors or Masters Degree.
  • License, Certification, Registration: Speech-Language Pathologist License (California) OR Physical Therapist License (California) OR Occupational Therapist License (California).
  • Additional Requirements: Demonstrated ability to plan and evaluate rehabilitation therapy programs; knowledge of local, state, federal, Medicare, and TJC standards/requirements pertaining to home health rehabilitation therapy services.
  • Preferred Qualifications: Two (2) years of supervisory or program management experience, or teaching experience within a university setting; five (5) years of home health experience; geriatric or comparable certifications via the APTA, AOTA, or ASHA professional associations; Doctoral Degree.
