Job Summary

We are seeking a qualified Licensed Practical Nurse to join our interdisciplinary team. As an LPN, you will play a vital role in delivering quality nursing care to patients and providing support to our healthcare team.

Key Responsibilities

  • Deliver high-quality patient care through interpersonal contact and provide services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being.
  • Monitor and report changes in patient condition, and provide clinical supervision to Certified Nursing Assistants (CNAs).
  • Assign duties, instruct, and provide guidance to CNAs to ensure seamless care delivery.
  • Collaborate with the healthcare team to develop and implement individualized care plans.

Requirements

  • Graduation from an accredited LPN program.
  • Current licensure/registration in applicable State, with a valid Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing.
  • One (1) year of skilled nursing experience preferred.
  • CPR certification upon hire or obtain during orientation, with current certification throughout employment.
