Position Overview:
The Certified Healthcare Assistant plays a vital role in collaborating with the medical team to ensure patients receive high-quality and comprehensive care.
Key Responsibilities:
- Effectively communicates health-related information to patients, support personnel, and fellow team members.
- Handles patient inquiries via phone, documenting and relaying questions in Electronic Health Records (EHR) to the provider as needed.
- Conveys pertinent information from providers to patients based on EHR documentation.
- Facilitates patient education in conjunction with healthcare providers.
- Coordinates and processes medication refill requests in collaboration with the provider.
- Monitors and manages voice mail, EHR messages, and emails promptly.
- Assists the care team in evaluating patients and identifying medical conditions.
- Coordinates patient care, referrals, and transitions in collaboration with other members of the Family Health Centers, Inc. care team.
Patient Care Duties:
- Delivers nursing care to patients in accordance with established standards and practices, adhering to the scope of the LPN license or MA-C certification.
- Records vital signs and patient medical history accurately.
- Administers medications as prescribed, including legend drugs, vaccines, and controlled substances, under the direction of a healthcare practitioner.
- Conducts diagnostic tests, including electrocardiography and respiratory assessments, as permitted under federal regulations.
- Prepares for and assists with medical procedures in line with clinic protocols and policies.
- Implements standing orders for patients as authorized by the Medical Director.
- Regularly checks and records refrigerator temperatures to ensure proper medication storage.
- Triages patients according to the level of licensure, assessing needs and providing appropriate nursing care.
Professional and Technical Skills:
The role requires a foundational level of written and verbal communication skills, computational abilities, and computer literacy, typically gained through high school education. Specific knowledge and training as a Licensed Practical Nurse and in Medical Assistance are essential, usually acquired through a formal education program lasting up to one year.
