About the Role:

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Care Coordinator. In this role, you will provide direct care and support to patients and their families, working closely with the interdisciplinary team to ensure optimal care experiences.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions between settings.
  • Administer medication and treatment as prescribed by the physician, and provide education to patients and families on self-care and disease management.
  • Provide emotional support and counseling to patients and families, addressing their physical, emotional, and spiritual needs.
  • Maintain accurate and detailed clinical notes, and communicate effectively with the healthcare team to ensure continuity of care.
  • Ensure compliance with all state and federal regulations, and participate in quality improvement initiatives to enhance patient outcomes.

Requirements:

  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • 1 year of nursing experience, with 3+ years preferred.
  • Experience in hospice or a similar setting is highly desirable.
  • Knowledge of the hospice philosophy of care, and a commitment to clinical and documentation excellence.
