  • Develop and implement individualized treatment plans for each patient, taking into account their unique needs and goals.
  • Communicate regularly with patients, families, and healthcare teams to ensure seamless care and coordination.
  • Conduct thorough assessments of patients' physical abilities and create personalized plans to address their needs.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for patients.

Requirements:

  • Graduate of an accredited Physical Therapy program.
  • Licensed as a Physical Therapist in Pennsylvania.
  • CPR Certified or ability to obtain.
  • Previous experience with Computer Charting.
  • 6 months of previous Home Health Care experience.
  • Valid U.S. Driver's License and reliable transportation.
