Geriatric Care Clinician

At TeamHealth, we're committed to providing high-quality care in local post-acute communities. As a Nurse Practitioner or Physician Assistant, you'll be responsible for delivering safe and cost-effective patient care with the support of our operations team.
Responsibilities- Work collaboratively with an interdisciplinary team to round on patients as medically necessary, conducting tasks such as H&P, admission, progress, transfer, discharge, and acute/follow-up assessments.
- Develop personalized plans for patient transfers to suitable care settings, including home, independent living centers, assisted living, and memory care facilities.
- Collaborate closely with hospice, palliative care, and home care agencies to ensure seamless transitions for patients.
We offer a productivity-based compensation model (Fee for Service) that allows you to maximize your income potential. Our estimated annual salary range is $85,000-$105,000, with no cap on additional earnings. This full-time role is benefit eligible, offering comprehensive benefits, paid professional liability insurance, and a 40-day onboarding program with MIPS training and support.
