We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare. As a key member of our healthcare team, you will provide high-quality patient care and support to our clients in the comfort of their own homes.

Responsibilities:
  • Provide direct patient care under the supervision of a registered nurse, following the developed care plan and physician orders.
  • Assess and document patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Implement and update the nursing care plan, taking appropriate nursing action based on assessment and achieving expected outcomes.
  • Provide education to patients and families regarding disease processes, self-care techniques, and prevention strategies.
  • Collaborate with the healthcare team to ensure seamless care delivery and achieve positive patient outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current State license, CPR certification, and valid driver's license (local travel required).
  • One (1) year of pediatrics experience (South Carolina) or one (1) year of experience working under current nursing license (California).
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
