The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) plays a crucial role within the outpatient environment, collaborating with a diverse healthcare team.
Role Overview:
- Provides primary healthcare to individuals across various age groups (family medicine) or specifically to adult patients (internal medicine).
- Conducts preventive health measures, including routine check-ups, screenings, and vaccinations, along with managing acute and complex medical issues that require diagnosis, treatment, and referrals to specialists when necessary.
- Works in conjunction with a multidisciplinary healthcare team.
- Evaluates and addresses patient risk factors, encompassing physical, socioeconomic, and psychosocial aspects.
- Gathers and records comprehensive patient histories.
- Performs and documents thorough physical examinations.
- Orders, executes, and interprets relevant diagnostic tests, addressing results that necessitate further evaluation.
- Develops prioritized differential diagnoses.
- Engages with collaborating physicians as needed (e.g., for uncertain diagnoses or unresponsive patients).
- Coordinates care with other specialists as required.
- Identifies anticipated outcomes from diagnoses and formulates a documented care plan addressing preventive, acute, and chronic health needs.
- Applies evidence-based guidelines and protocols in a tailored, dynamic care plan suitable for various healthcare contexts.
- Adjusts care plans based on patient responses, diagnostic results, and recommendations from specialists.
- Prescribes diagnostic and therapeutic interventions as necessary.
- May perform in-office procedures in line with established competencies.
- Documents outpatient visits in the EPIC system utilizing the SOAP format.
- Ensures timely documentation in Epic, adhering to compliance standards and capturing patient history, presenting issues, physical exams, diagnoses, and treatment plans.
- Completes and signs visit notes within 24 hours of patient encounters.
- Reviews diagnostic study results promptly and communicates follow-up plans to patients.
- Addresses telephone and Epic portal messages as needed.
- Reviews specialist consultations and other relevant patient information.
- Provides coverage for In-Basket communications when colleagues are unavailable.
- Maintains ongoing communication with office staff and the interdisciplinary healthcare team.
- Facilitates communication with patients and the care team.
- Reports any compliance or patient safety concerns.
- Collaborates with the population health team to enhance patient access and outcomes.
- Strives to meet annual quality metrics established by Jefferson Medical Group.
- Achieves target productivity levels, assessed through work RVUs (relative volume units).
- Exhibits professional behaviors, including mentoring and educating graduate students.
- Seeks opportunities for involvement in research and evidence-based practice analysis.
- Completes mandatory annual learning activities.
- Actively participates in Advanced Practice Grand Rounds and specialty meetings.
- Maintains national board certification, state licensure, and required continuing education, along with membership in relevant professional organizations.
- Master's Degree
- National Board Certification as a Primary/Family Care Nurse Practitioner or Physician Assistant
- BLS Certification
- ACLS Certification
- Proficient in English, Vietnamese, and Spanish
Conditions of Employment:
Covid Vaccination is mandatory for employment at Jefferson for individuals working in clinical settings.
