Job Summary

In this role, you will work under the direct supervision of the Director of Care Management and the Lead Social Worker II to provide assessment and referral for services related to complex care needs. You will consult with patients and families to adjust to illness, intervene in crises, and preserve patient self-determination in decision-making processes.

Responsibilities
  • Conduct comprehensive assessments of patients' behavioral, psychological, and socioeconomic needs, identifying required resources throughout the care continuum.
  • Attend care planning and discharge planning rounds to facilitate communication and emphasize patient goals.
  • Participate in advance care planning initiatives and collaborate with the team using leadership and communication skills.
  • Evaluate and respond to client, clinician, and community needs, reviewing decisions with the Lead/SW II when necessary.

Requirements

The ideal candidate will have 3-5 years of clinical social work experience, preferably in acute care settings. A Bachelor's or Master's degree in Social Work is required. The salary range for this position is $ 62,000 - 85,000.
