**Job Summary**
The Behavioral Health Navigator at Sentara Healthcare serves as a liaison between patients, healthcare providers, and community resources. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
**Key Responsibilities**
- Assess patients' behavioral health needs and develop individualized care plans.
- Collaborate with healthcare teams to ensure seamless transitions of care.
- Provide education and support to patients and their families on mental health conditions and available resources.
- Identify and address barriers to treatment, such as financial constraints or lack of transportation.
**Requirements**
- Two years of experience in case management or long-term care.
- Bachelor's degree in health & human services or equivalent experience.
- LCSW or LPC licensure may be required for certain positions.
