About the Role:

Aequor Technologies is seeking a skilled Physical Therapist to join our team in providing in-home physical therapy services to patients recovering from illness, surgery, or injury. As a Physical Therapist in our Home Health program, you will conduct comprehensive assessments of patients' physical abilities, mobility, strength, balance, and functional limitations to develop personalized treatment plans. You will design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion. Additionally, you will educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries in the home. You will collaborate with physicians, nurses, and other members of the healthcare team to ensure a coordinated and comprehensive approach to patient care. Regularly, you will assess and monitor patient progress, adjusting treatment plans as needed to ensure optimal recovery and achievement of therapy goals. You will document patient evaluations, treatment plans, and progress notes in the electronic health record (EHR), ensuring compliance with home health regulations and standards. Furthermore, you will provide recommendations for adaptive equipment, such as walkers or grab bars, to support patient mobility and safety at home. Participating in discharge planning, you will ensure patients receive continued support or outpatient therapy services as needed after home health care. To excel in this role, you must have a strong understanding of physical therapy principles, rehabilitation techniques, and home health care protocols. You must also possess excellent communication and interpersonal skills for working with patients, families, and healthcare teams in a home health environment. You should be proficient in conducting assessments, creating individualized therapy plans, and implementing physical therapy interventions. An active Physical Therapist (PT) License in good standing is required, as well as a minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting. A completion of an accredited physical therapy program (Master's or Doctoral degree) and certification in Basic Life Support (BLS) are also necessary. Experience with home health documentation systems (e.g., EPIC, Homecare Homebase, etc.) is a plus.
