About the Position:
We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our Home-based Medical Care team in Windham, CT. As a member of this team, you will have the opportunity to make a meaningful difference in the lives of patients and their families.
Key Responsibilities:
- Provide high-quality primary care to patients with complex needs, including geriatric and palliative care
- Develop and implement individualized patient care plans in collaboration with interdisciplinary teams
- Conduct regular assessments and evaluations to monitor patient progress and adjust care plans accordingly
- Prescribe medications and treatments as necessary to improve patient outcomes
- Participate in quality improvement initiatives to enhance patient care and satisfaction
- Stay up-to-date with the latest medical research, technologies, and best practices to provide evidence-based care
Requirements:
- Master's degree in Nursing or a related field
- Current RN or NP/PA license in good standing
- Certification in a relevant specialty (e.g., Gerontology, Palliative Care)
- Experience working with complex patients and their families
