About the RoleAs a LPN/LVN, you will be responsible for assessing, planning, implementing, and evaluating patient care. You will work closely with our team to provide comprehensive care to our patients, including those with gastrostomy tubes, tracheostomy tubes, and mechanical ventilators.
Responsibilities- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
- Perform other LPN/LVN duties as assigned.
- Current Licensed Practical Nurse (LPN/LVN) in the state in which the LPN/LVN practices.
- Current Health Certificate (per facility).
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior LPN/LVN experience preferred.
