Job Summary

We are seeking a skilled Registered Nurse to provide high-quality patient care and coordination in a home health setting. As a Nurse Care Coordinator, you will be responsible for delivering and documenting skilled nursing care in accordance with developed care plans and physician orders.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
  • Develop, implement, and update the nursing care plan
  • Take appropriate nursing action based on assessment and achieve expected outcomes
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current, unrestricted state license as a Registered Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
  • One year of experience required working under current nursing license
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Compensation and Benefits

The estimated salary for this position is $27-32 per hour. We offer a competitive compensation package, including benefits such as paid time off, health insurance, and retirement savings plan.
