Job Summary

Bridgewell is seeking a highly skilled Clinical Case Manager to join our Women's Substance Abuse Services team. As a Clinical Case Manager, you will provide individual and group counseling to residents, working closely with a caseload of up to eight clients. Your primary focus will be on supporting dual-diagnosed or high-risk clients, ensuring they receive the necessary care and support to achieve their full potential.

Key Responsibilities
  1. Complete Clinical Intake within the first week of admission
  2. Develop and update Treatment Plans within the first two weeks of admission
  3. Utilize the electronic Medical Record to document client progress and update records as needed
  4. Meet weekly with assigned individuals to discuss progress and goals
  5. Participate in Clinical Supervision to ensure adherence to best practices and regulatory requirements
  6. Input entries into the Medical Record in accordance with service line procedures
  7. Participate in the on-call rotation to provide 24/7 support to clients
  8. Expand knowledge and participate in training to enhance clinical skills and stay up-to-date on industry developments
  9. Understand and adhere to DPH regulations and CARF standards as applicable
  10. Provide transportation as needed to support client engagement and participation
  11. Establish and maintain relationships with DCF, Guardian, doctors, and therapists to ensure comprehensive care
  12. Make outside referrals as needed to support client well-being
  13. Attend weekly staff meetings to stay informed and contribute to team discussions
  14. Provide weekend, holiday, and emergency coverage as required
  15. Perform other duties as assigned by management
Requirements
  1. A minimum of a High School Diploma or GED is required
  2. Bachelor's degree or clinical licensure is preferred
  3. A minimum of five years of experience in the field is required
  4. Individual and group counseling experience is essential
  5. A valid MA driver's license is required
Preferred Qualifications
  1. Strong written and verbal communication skills
  2. Ability to maintain accurate and efficient records
  3. Capacity to respond to crisis situations
  4. Two years of continuous sobriety (if recovering)
  5. Ability to work independently and as part of a team
