Clinical Case Manager

Bridgewell is seeking a highly skilled Clinical Case Manager to join our Women's Substance Abuse Services team. As a Clinical Case Manager, you will provide individual and group counseling to residents, working closely with a caseload of up to eight clients. Your primary focus will be on supporting dual-diagnosed or high-risk clients, ensuring they receive the necessary care and support to achieve their full potential.
Key Responsibilities- Complete Clinical Intake within the first week of admission
- Develop and update Treatment Plans within the first two weeks of admission
- Utilize the electronic Medical Record to document client progress and update records as needed
- Meet weekly with assigned individuals to discuss progress and goals
- Participate in Clinical Supervision to ensure adherence to best practices and regulatory requirements
- Input entries into the Medical Record in accordance with service line procedures
- Participate in the on-call rotation to provide 24/7 support to clients
- Expand knowledge and participate in training to enhance clinical skills and stay up-to-date on industry developments
- Understand and adhere to DPH regulations and CARF standards as applicable
- Provide transportation as needed to support client engagement and participation
- Establish and maintain relationships with DCF, Guardian, doctors, and therapists to ensure comprehensive care
- Make outside referrals as needed to support client well-being
- Attend weekly staff meetings to stay informed and contribute to team discussions
- Provide weekend, holiday, and emergency coverage as required
- Perform other duties as assigned by management
- A minimum of a High School Diploma or GED is required
- Bachelor's degree or clinical licensure is preferred
- A minimum of five years of experience in the field is required
- Individual and group counseling experience is essential
- A valid MA driver's license is required
- Strong written and verbal communication skills
- Ability to maintain accurate and efficient records
- Capacity to respond to crisis situations
- Two years of continuous sobriety (if recovering)
- Ability to work independently and as part of a team
