Position Title: Clinical Case Manager
Overview:
The Family and Children's Association is in search of a dedicated Case Manager for our Linkage Program. This program is designed to offer short-term case management, counseling, education, and connections to long-term care and community resources for individuals aged 55 and older who are experiencing behavioral health issues, substance use challenges, and difficulties related to aging.
Key Responsibilities:
- Deliver a combination of telehealth services and in-home visits to clients, working closely with program staff to meet client needs.
- Maintain comprehensive client case records, assessments, and statistics in compliance with the standards established by Family & Children's Association and relevant state guidelines.
- Document billable services in an electronic health record system.
- Provide case management support by assisting clients with applications for services, offering information and referrals, and addressing barriers to accessing resources.
- Accompany clients to appointments for support and advocacy, representing them in situations where they cannot advocate for themselves.
- Engage in regular supervision with the Program Coordinator and participate in weekly team meetings, as well as agency training sessions.
Qualifications:
- Experience working with the geriatric population or individuals with serious mental illness is preferred.
- A Master's degree in a related field is required (LMSW or limited permit is preferred).
- A valid driver's license in New York State is necessary.

Work Schedule:
- Full-time position, requiring 35 hours per week, Monday through Friday.
