• Conduct thorough assessments and develop personalized rehabilitation plans for patients
• Provide one-on-one physical therapy services in patients' homes, focusing on functional outcomes and patient-centered care
• Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
• Stay up-to-date with industry developments and best practices through ongoing education and training
• Contribute to a positive, supportive team environment that fosters growth and development

• Valid Physical Therapy license in the state(s) of practice, or eligibility to apply
• Degree from an accredited physical therapy program
• Basic computer literacy skills
• Current CPR certification
