Job Summary

We are seeking a skilled Discharge Planner to join our team at WVU Medicine. As a key member of our interdisciplinary treatment team, you will be responsible for ensuring a smooth transition for patients from our care to their next destination.

Key Responsibilities
  • Conduct comprehensive assessments to identify patient needs and develop individualized discharge plans
  • Collaborate with the treatment team to coordinate care and ensure a seamless transition for patients
  • Communicate effectively with patients, families, and healthcare providers to ensure a smooth discharge process
  • Document patient progress and discharge plans in a timely and accurate manner
Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, Nursing, or related field
  • Valid driver's license
  • West Virginia state criminal background check, and federal, if applicable, required for DHHR regulated area
Preferred Qualifications
  • One (1) year related experience in a mental health program preferred in a setting providing acute and/or residential psychiatric care
Work Environment

The work environment includes time in the office as well as walking around interacting with patients and staff. You will also interact and work with trainees of multiple disciplines.
