The Advanced Practice Nurse is primarily responsible for performing clinical evaluations of health plan members, documenting and coding interactions in the authorized electronic medical record systems. This role aims to enhance our members' quality of life and facilitate optimal health outcomes through efficient healthcare practices. The NP will report to the NP Manager.
Key Responsibilities:
- Conduct comprehensive assessments in various environments for health plan members, including reviewing medical history, symptoms, physical examinations, medication assessments, and depression screenings.
- Responsible for measuring vital signs and performing physical examinations, including monofilament tests and foot examinations as necessary.
- Identify diagnoses to guide care management and active medical treatment.
- Compile a detailed list of current and past medical conditions using clinical expertise and the results of your evaluations.
- Communicate assessment findings to inform the Primary Care Provider of potential care gaps.
- Educate members on topics such as disease management, medication adherence, and compliance.
- Adhere to all HIPAA regulations and ensure the security of protected health information (PHI).
- Since this role may involve home visits, the ability to work independently and travel extensively is required. Comfort in working with geriatric patients with diverse health issues is essential.

Required Qualifications:
- Active, unrestricted nursing license in the assigned state.
- Nurse Practitioner Certification from ANCC, AANP, or AACN in Adult, Geriatric, Family, or Acute Care NP.
- Minimum of 1 year of clinical experience in the highest level of education.
- Willingness or ability to spend at least 1 hour with a member in their home, if applicable, which may include remote areas and environments with pets or tobacco users.
- Willingness or ability to lift a 30-pound bag in and out of a vehicle and navigate stairs and various living conditions.
- Access to reliable transportation to travel to client and/or patient locations within a designated area.
Preferred Qualifications:
- Experience in Gerontology, Cardiology, Internal Medicine, Endocrinology, or Family Medicine.
- Experience in home care or home visits.
- Proficient in computer use and able to navigate the internet.
- Demonstrated excellent administrative and organizational skills with the ability to communicate effectively with seniors and their families.
