Job Overview

We are seeking a highly skilled Dietary Therapist to join our team at Chi. As a key member of our department, you will be responsible for providing Medical Nutrition Therapy to patients with medical conditions requiring dietary modification. This role involves participating in overall program development and implementation, quality improvement, and promotional activities of the department.

Key Responsibilities:
  • Conduct thorough assessments of patient education needs and develop individualized education plans.
  • Provide initial and follow-up consultations for diabetes patients and others requiring dietary modifications, offering personalized guidance on nutrition and lifestyle management.
  • Teach Diabetes diet principles, disease process, and treatment in group and individual sessions, ensuring patients have a comprehensive understanding of their condition.
  • Evaluate and develop teaching tools for use in education activities, continuously improving our resources to better support patient learning.
  • Assist patients with setting goals to achieve optimal diabetes self-management and monitor patient outcomes, celebrating successes and addressing challenges along the way.
  • Communicate effectively with health team members regarding patient education and outcomes, fostering collaboration and mutual support.
  • Participate in departmental continuous quality improvement projects, including patient chart reviews, to identify areas for growth and implement positive changes.

Qualifications: A Bachelor's Degree in Nutrition or Dietetics is required, and registration as a Registered Dietitian is essential. A Master's degree in a related field is preferred, demonstrating advanced knowledge and skills in nutrition therapy and education. With a strong passion for patient-centered care and a commitment to staying up-to-date with industry developments, you will excel in this role.
