Key Responsibilities:



  • Cultivate health education and services in collaboration with physicians, healthcare staff, and other providers.
  • Deliver patient education and information to manage chronic health conditions in group or individual settings.
  • Participate in diabetes self-management education planning, implementation, and evaluation.
  • Notify supervisors or physicians of patient reactions to treatments and significant incidents.
  • Initiate assessments and education according to patient needs, as prescribed by physicians and/or policy.
  • Develop and disseminate educational materials.
  • Contribute to health education program development and marketing.
  • Participate in multidisciplinary team meetings.
  • Maintain educational resources.

Qualifications:



  • Bachelor's degree in Food and Nutrition, Dietetics, Nutritional Sciences, or a related field.
  • Completion of a coordinated program or dietetic internship of at least 900 hours under a Registered Dietitian.
  • Desirable experience in chronic disease management programs.

Language and Problem-Solving Skills:



  • Ability to read, analyze, and interpret professional materials.
  • Ability to write reports and business correspondence.
  • Ability to present information and respond to questions.

Certifications and Licenses:



  • Licensed and Registered Dietitian in Texas.
  • Preferred Certified Diabetes Educator or certification within two years of employment.

Computer Skills:



  • Working knowledge of the Internet, Outlook, PowerPoint, Word, Excel, and electronic health records software.

Work Environment and Physical Demands:



  • Medical office environment with patient interaction.
  • Bending, stooping, and lifting at least 25 pounds.
