We are seeking a skilled Physical Therapist to join our team at an upscale Continuum Care Community in Stone Mountain, GA.

Key Responsibilities:

  • Develop and implement effective treatment plans to optimize patient functionality and skills.
  • Evaluate and treat residents after receiving written orders from physicians.
  • Screen facility residents and complete necessary documentation.
  • Attend required meetings and in-services, including family meetings, care plans, staffing, and utilization review.
  • Collaborate with the Director of Rehabilitation to develop programs tailored to the needs of facility residents.
  • Provide consultation and clinical supervision to Physical Therapy Assistants.
  • Maintain valid state licensure and awareness of issues related to the profession of PT and the healthcare environment.
  • Responsible for scheduling residents and facilitating family and facility staff in-services and education.
  • Assist the facility in meeting standards necessary to maintain state and federal certification.

Requirements:

  • Master of Science Degree or greater in Physical Therapy.
  • State licensure.
  • Excellent communication skills, both written and verbal.
  • Ability to maintain professionalism in all situations.
  • Strong time management and ability to manage caseload.
  • Ability to clinically supervise Physical Therapy Assistants according to state regulations.

Preferred Qualifications:

  • Senior Living experience.
  • PDPM knowledge.
  • Experience with therapy software/EMR.
  • Medicaid case mix knowledge.
