Key Responsibilities- Conduct thorough assessments of patients' mental health needs and develop personalized treatment plans.
- Provide direct clinical services to patients of all ages, including those with diverse backgrounds and needs.
- Utilize evidence-based screening tools to identify patients at risk for mental illness and develop strategies for prevention and intervention.
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
- Document all assessments, interventions, and treatment plans in accordance with departmental and clinic guidelines.
- Participate in multidisciplinary teams to promote integration between primary care and behavioral health services.
- Masters degree in social work from an accredited university, with two years of post-graduate experience preferred.
- Registered with the Board of Behavioral Sciences and working towards licensure.
- Bilingual in English and Spanish is required.
- Completion of 1500 hours towards licensure.
