POSITION SUMMARY:

The Licensed Practical Nurse (LPN) operates within the Patient Care Services framework as an integral member of the healthcare team.

The primary duties of the LPN include providing patient care under the supervision of a Registered Professional Nurse (RPN) in accordance with the New York State Practice Act.


QUALIFICATION REQUIREMENTS:
Assist patients in achieving outcomes as determined by the RPN in the care plan.

The LPN is responsible for gathering and documenting data regarding the physiological, cognitive, cultural, and spiritual status of patients, assisting in the development of evidence-based, age-appropriate care plans, and implementing care while adhering to established standards that define the structure, process, and desired outcomes of the nursing process. The LPN also communicates information that requires referral to the RPN and reinforces the teaching plan established by the RPN.

To excel in this role, an individual must successfully perform each essential duty as outlined in the annual competency assessment.

The LPN adheres to unit and human resource policies, contributes to the development and achievement of unit/service and patient care goals, and organizes the delivery of patient care efficiently and cost-effectively.

The LPN takes responsibility and accountability for professional standards and personal practice, attends required Inservice Education Programs, maintains role-specific annual competency, recognizes and communicates situations that require referral to an RPN, and interacts with patients, families, significant others, and coworkers with respect and kindness.

The LPN maintains confidentiality of information, demonstrates the necessary knowledge and skills to provide patient care appropriate to the age and special characteristics of the patients served, and performs data collection and documentation of interventions within the protocol or care plan.

The LPN may not initiate care plans or protocols, complete 24-hour analyses or end-of-shift summaries, or provide end-of-shift reports.

Provide technical support as demonstrated by knowledge and validated competency skills related to:

- Medication administration
- Assisting and providing care to clients with peripheral IV Therapy (no central line devices)
- Performing phlebotomy
- Safety measures
- Personal hygiene skills
- Nutritional skills
- Elimination skills
- Heat and cold applications
- Admission, transfers, discharge
- Perioperative care
- Musculoskeletal skills
- Respiratory skills
- Transfer/transport skills
- Specimen collection/testing.

Ensure Safety and Cleanliness of Environment, Equipment, and Supplies by maintaining a clean and safe environment to meet quality patient care standards.

The LPN is responsible for cleaning up after completing work in common areas, monitoring hallways and common areas to ensure equipment is properly placed, and cleaning equipment.

LPN IIIs may, under the direct supervision of an RPN, monitor (not initiate) blood product administration via peripheral routes only, initiate Peripheral Intravenous Therapy, administer pre-medicated IV fluids, and flush intermittent peripheral venous access devices with normal saline solutions to maintain patency.


EDUCATION:
Must be a graduate of an accredited School of Practical Nursing.

Must hold current New York State registration or possess a limited permit to practice in New York and current BLS certification.

Based on unit-specific criteria

COMMUNICATION SKILLS:
Ability to read and interpret documents such as safety rules and procedure manuals. Ability to document patient care on established forms. Ability to communicate effectively and cooperatively with patients, family members, employees, and others. Ability to listen attentively, take direction, and engage in interactive dialogues. Ability to seek input from others to achieve consensus.

MATHEMATICAL SKILLS:

Ability to perform basic arithmetic operations accurately, including addition, subtraction, multiplication, and division with whole numbers, common fractions, and decimals, particularly in medication administration.

Ability to compute rates, ratios, and percentages.



REASONING ABILITY:
Ability to apply common sense understanding to carry out instructions provided in written, oral, or diagram form. Ability to address problems involving several concrete variables in standardized situations.

While performing the duties of this job, the employee is regularly required to stand, walk, use hands to probe, handle, or feel objects, tools, or controls, reach with hands and arms, and speak and hear.

The employee may occasionally be required to sit and stoop, kneel, or crouch. The employee must regularly lift and/or move up to 50 pounds and frequently lift and/or move more than 50 pounds.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

While performing the duties of this job, the employee may be exposed to infectious materials and chemicals.

The noise level in the work environment is typically moderate. This job requires that the employee be physically on-site as the work cannot be performed remotely.
