Job Summary:We are looking for a compassionate and dedicated Registered Nurse to provide skilled nursing care to patients in the home setting. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.
Responsibilities:- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Registered Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Registered Nurse in a clinical care setting.
- Home health experience.
