Job Summary: Della Infotech, Inc. is seeking a seasoned LVNLPN to fill a Medical Care Professional role at our Claremont, NH facility. In this position, you will provide direct patient care, work collaboratively with the interdisciplinary team, and contribute to the overall success of our organization. Key Responsibilities:
  • Provide direct patient care, including administering medications, treatments, and wound care
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Communicate effectively with colleagues, residents, and family members to ensure seamless care transitions
  • Participate in ongoing education and training to enhance clinical skills and knowledge
  • Maintain accurate and detailed records of patient care, including MARs/TARs and electronic documentation

Requirements:
  • 2+ years of experience as an LVNLPN
  • Valid New Hampshire RN license or LPN license in compact state
  • BLS certification (AHA)

Working Conditions:
This is a full-time position that requires working every other weekend and holiday coverage. On-call requirements are not applicable for this role. You must be willing to live within 50 miles of the facility and be available to start immediately.
