About the Role:

We are seeking a compassionate and skilled Medical Social Worker to join our team at Signature Healthcare at Home. As a Hospice Medical Social Worker, you will play a vital role in providing emotional and practical support to patients and their families during a challenging time.

Key Responsibilities:

  • Assess the psychosocial status of patients and their families, identifying areas of need and developing plans to address them.
  • Provide counseling and support to patients and their families, helping them cope with stress and other emotional challenges.
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Maintain accurate and detailed clinical records, documenting patient interactions and progress.

Requirements:

  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or Certified Social Worker in Administration (CSWA) in Oregon.
  • Minimum one year of experience in healthcare, with a preference for hospice experience.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
