Job Title: Licensed Practical Nurse - Private Duty Nursing

Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality, one-on-one nursing care to our patients in the comfort of their own homes. As a Private Duty Nursing professional, you will work closely with our team to deliver compassionate and personalized care to individuals with complex medical needs.

Responsibilities:

- Provide skilled nursing care to patients with tracheostomies and ventilators
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with our interdisciplinary team to ensure seamless care coordination
- Maintain accurate and detailed records of patient care and progress

Requirements:

- Current LPN license in good standing
- CPR certification (in-person training only)
- TB skin test (current within the last 12 months)
