About the Role

Hartford HealthCare's Neighborhood Health program is seeking passionate per diem Advanced Practitioners to join our primary care team dedicated to improving health equity in underserved communities.

This role offers the opportunity to provide high-quality preventative and urgent care through mobile and community-based services, addressing health disparities and promoting wellness.

Key Responsibilities

  • Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
  • Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
  • Provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Requirements

  • Unrestricted State of Connecticut Medical License (or eligible)
  • Family Nurse Practitioner (FNP) or Physician Assistant (PA) training
  • 2+ years of experience as a Family Nurse Practitioner or Physician Assistant, preferably in a primary care or urgent care setting
