Sage Health is seeking a Primary Care Physician to join our team in Waverly/Baltimore. As a key member of our care team, you will have the opportunity to work with a diverse group of patients and provide high-quality, patient-centered care.
Responsibilities- Evaluate and treat patients in accordance with standards of care
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care
- Participate in daily huddles with the care team to discuss patient care and coordinate services
- Work with the Lead Physician and Center Administrator to set a tone of cooperation in practice
- Complete medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation
- Follow policy and protocol defined by Clinical Leadership
- Meet with the Lead Physician to discuss quality of care, review outcome data, and medical records issues
- Graduate of an accredited MD or DO program
- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred
- Active and unrestricted DEA license
- Medicare Provider Number
- Medicaid Provider Number
- Strong interpersonal and communication skills
- Ability to work in a team-based care model
- Willingness to learn and adapt to a value-based care setting
- Knowledge of HEDIS/STARS
- Knowledge of CAHPS and HOS
- Experience working in medically underserved or culturally diverse communities
- Experience working with an electronic health record
- Experience working in Medicare Advantage Global Risk Models
