Job Summary

Sage Health is seeking a Primary Care Physician to join our team in Waverly/Baltimore. As a key member of our care team, you will have the opportunity to work with a diverse group of patients and provide high-quality, patient-centered care.

Responsibilities
  • Evaluate and treat patients in accordance with standards of care
  • Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care
  • Participate in daily huddles with the care team to discuss patient care and coordinate services
  • Work with the Lead Physician and Center Administrator to set a tone of cooperation in practice
  • Complete medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation
  • Follow policy and protocol defined by Clinical Leadership
  • Meet with the Lead Physician to discuss quality of care, review outcome data, and medical records issues
Requirements
  • Graduate of an accredited MD or DO program
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice
  • Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine preferred
  • Active and unrestricted DEA license
  • Medicare Provider Number
  • Medicaid Provider Number
  • Strong interpersonal and communication skills
  • Ability to work in a team-based care model
  • Willingness to learn and adapt to a value-based care setting
Preferred Qualifications
  • Knowledge of HEDIS/STARS
  • Knowledge of CAHPS and HOS
  • Experience working in medically underserved or culturally diverse communities
  • Experience working with an electronic health record
  • Experience working in Medicare Advantage Global Risk Models
