Position Title: Certified Home Health Aide
Shift Details: Flexible full-time 12-hour shifts available, including both day and night options.
Role Overview: The Home Health Aide is responsible for delivering personal care and associated services within the home environment. This position operates under the guidance and supervision of nursing staff and the Director of Nursing.
Key Responsibilities:
- Assist clients with Activities of Daily Living (ADLs) as outlined in the Plan of Care (POC).
- Adhere to the POC for each individual and maintain accurate documentation.
- Support clients in maintaining personal hygiene and grooming.
- Prepare and serve nutritious meals tailored to client needs.
- Assist clients with mobility and ambulation as specified in the POC.
- Collaborate with therapy personnel to support rehabilitation efforts.
- Provide reminders for self-administered medications as assigned.
- Monitor and record vital signs in accordance with the care plan.
- Assist with treatments as prescribed by physicians, under nursing supervision.
- Document visit reports on the same day of service and update clinical records as required.
- Communicate any changes in client condition or home environment to the supervising nurse or aide supervisor.
- Execute assignments as directed by nursing staff and report any challenges encountered.
- Perform routine housekeeping tasks to ensure a safe and comfortable living space for clients.
- Participate in required in-service training sessions as mandated by state and federal regulations.
- Coordinate weekly scheduling of visits with the Supervisor/Director to ensure comprehensive care delivery.
