We are seeking a compassionate and skilled Registered Nurse (RN) to join our team at Linden Grove Health Care Center. As a RN, you will provide high-quality patient care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each resident.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
- Document patient care and progress, maintaining accurate and up-to-date records.
- Current RN licensure in the state of [insert state].
- Experience in a skilled nursing facility (SNF) or long-term care setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to lift, transfer, and reposition patients, as needed.
- Basic Life Support (BLS) certification.
