We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager at Middletown Park Rehabilitation & Healthcare. As a key member of our nursing staff, you will be responsible for providing high-quality patient care and ensuring the well-being of our residents.
Key Responsibilities- Provide Direct Patient Care: Administer medications, treatments, and perform various nursing procedures as needed.
- Assess and Document Patient Needs: Conduct thorough assessments of patients' physical and emotional needs, documenting findings and developing individualized care plans.
- Collaborate with Interdisciplinary Team: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Monitor and Evaluate Patient Progress: Regularly assess patients' progress, making adjustments to care plans as needed.
- Communicate Effectively: Maintain open and respectful communication with patients, families, and colleagues.
- Education and Licensure: Graduate of an approved school of nursing and licensed and registered in good standing in New York State as an RN.
- Experience and Skills: Previous experience in geriatric care is desirable, but not required. New grad RNs welcome to apply. Possess a strong spirit of cooperation and enthusiasm for creating a supportive environment for patients and colleagues.
- Knowledge and Abilities: Knowledge of medical, social, and psychological needs of residents, general nursing theory and practice, and infection prevention principles and methods. Ability to communicate effectively with patients, families, and staff.
