Overview: Liberty Health is seeking an experienced Home Care Physical Therapy Assistant to join our team. This role is responsible for providing patient care services, updating care plans, and coordinating patient schedules.
Job Summary: As a Home Care Physical Therapy Assistant, you will work closely with Physical Therapists to provide direct treatment and follow-up to patients. Your responsibilities will include updating care plans, planning patient schedules, and communicating with patients and their families.
Key Responsibilities:
- Patient Care Services: Provide patient care services along with direct treatment and follow-up as assigned by the Physical Therapist.
- Care Plan Updates: Update the care plan in coordination with the therapist and the physician, and plan with the patient/family toward adjustment.
- Schedule Coordination: Plan monthly patient re-evaluations with the therapist, coordinating with the therapist more frequently if needed, and plan the weekly patient schedule.
- Documentation: Document patient care data accurately, thoroughly, and in a timely matter, reflecting evidence of improvement/progress/decline in patient's condition.
Requirements:
- Credentials: Requires completion of an accredited physical therapy assistant program and current State licensure.
- Experience: Minimum of one year's therapeutic experience in a non-home health care environment; prefer two years of experience.
- Communication Skills: Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to treat patients for whom a physician's order exists.
